Request a Program with OHP

Want to program with the Office of Health Promotion? Complete our program request form located on The Hub. There are options to:

• Request OHP staff to provide resources, flyers, or other materials
• Request OHP staff to participate in an event planned by person/organization issuing the request, or
• Request OHP staff provide a presentation and/or facilitate a group activity
